Subject Access Requests (SARS) - Requesting Your Information

What is a Subject Access Request (SARS)?

A SARS is a request for any data that is held about you at an organisation. 

You have the right to make a request to any organisaion that holds your information for copies of your data.

How long does it take to receive your data?

Once we have received your request, we aim to provide your information as soon as possible. However, requests can take up to 28 days to be completed.

 

How do I make a request?

You can request your data using the following options:

NHSApp: You can access your full record online. click here to register for online services

viewing your record via the NHSApp is the fasest way to access your data.

In person: you can visit reception who will provide you with the correct form to complete.

Online: please click here to submit a request online

Please note, you may be asked to present ID before we can action your request.

If someone else is requesting information on your behalf, we will need to seek consent before we can proceed with the request.
